Grant Proposals
Agencies may submit proposals only if they have previously submitted a letter of inquiry and been invited to submit a full proposal.
Proposals may be submitted in traditional hard copy or online. Regardless of the format, the deadlines are the same. For the spring grant cycle, the deadline for submitting proposals is February 28. For the fall grant cycle, the deadline is August 31. (Please see the “Application Requirements and Deadlines” page for additional details about these deadlines.)
For instructions about online proposal submission, please go to Submitting an Application Online. Applicants submitting a hard copy should submit a single, unbound copy of the proposal, which should include:

Narrative.

A description of your organization, including the organization's history,
purpose, current services and programs, staff, mission, and website address. (2
pages maximum)

A summary of the proposed grant, its goals and objectives, plans for
implementation, population and numbers expected to be served, objectives for
the program and the information you will collect to monitor your progress. (3
pages maximum)

The specific amount requested, other sources of funding, and whether this is a
collaborative project (if so, describe other projected funding sources). (1
page maximum)

Financial summary. Each applicant must complete a three-page
financial summary form, which is available on this site or from the
Foundation office.

Attachments. Each application must include a
grant proposal checklist, also available on this website or the
Foundation office, showing that your organization is submitting the following
attachments:

Staff and qualifications of those involved in the proposed project.

List of directors, trustees and officers.

A project budget, including income and expenses.

If your agency does have an audit, then please provide:

Most recent audited financial statements
(including statement of
financial position, statement
of activities, statement of
cash flows, and, if available,
statement of functional expenses). If your agency received a
management letter
along with your audit, please submit a copy of that letter as well.

Unaudited statement of
financial position and statement
of activities for the period beginning at the end of the fiscal year
covered by the most recent audit, and ending at the last fiscal quarter before
you submit your proposal. For example, if you have an audit for the fiscal year
ending December 31, 2009, and you are submitting a proposal in August 2011, you
should provide unaudited financial statements trhough June 30, 2010.
- Operating budgets for the year covered by your audit and your current fiscal year. Please also provide your operating budget for the year in between, if any. For example, your audit is for the fiscal year ending 12/31/09. You are submitting a proposal during your 2011 fiscal year. Please submit operating budgets for fiscalyears 2009, 2010, and 2011

If your agency does not have an audit, then please provide:

Please check the nonprofit website www.guidestar.org,
to see if your agency’s most recent informational tax return,
Form 990, has been posted there. If it has not been, please send us the
public inspection copy of your most recent return, including
Schedule A. If you do not file Form 990, please include a brief
explanation why your agency is exempt from the filing requirement.

Optional material supporting the grant request. (4 pages maximum). (Agencies
are encouraged not to include CDs, DVDs, or videotapes with their requests
unless specifically requested by the Foundation.)
